US CareNet Announces HC360 Technologies

US CareNet, a national senior care services provider based in Augusta, Ga., announced that they have purchased controlling interest in the technology used in their NavCare care management division, forming a new company doing business as HC360 Technologies.

"NavCare needed a platform that would address the challenges and continuous changes of healthcare requirements," said Gary Liuzzo, Vice President of Information Technology for US CareNet. "HC360 Technologies is positioned to deliver an advanced, cloud-based solution with integrated Voice over Internet Protocol."

The newly enhanced platform provides for Chronic Care Management, Transitional Care Management services, and Population Health Management services, as well as other care coordination services. In January 2015, the Centers for Medicare and Medicaid Services began reimbursing providers who actively manage care delivery for Medicare patients suffering from two or more chronic conditions. These services may be contracted with a third party, such as US CareNet's NavCare division.
